In consideration of the privilege granted me to participate in the Arizona Veterans Fly Fishing, Inc (“AVFF”) programs (the “Programs”), I, the undersigned, on behalf of myself, my next of kin, my personal representatives, my heirs, my executors, and my administrators hereby acknowledge, agree, and represent, that: 

I hereby choose to accept the risk of injury or the contraction of serious and potentially life- threatening conditions or illnesses, long term disability, and even death in order to participate in the Programs. These services are of such value to me that I accept the risks of injury, exposure, or contraction of known or unknown illnesses or conditions resulting from in person participation in the Programs. This includes participation in meetings and other organizational activities including, but not limited to, fly tying, rod building, water certification, casting and fishing trips. 

I fully understand and appreciate both the known and unknown potential dangers associated with participation in the Programs and acknowledge that my participation may, despite AVFF’s reasonable efforts to mitigate such dangers, result in injury, serious illness, disability and/or death.

ASSUMPTION OF RISK: I have read and fully understand the above warnings and hereby choose to accept any risks associated with participation in the Programs.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to sue AVFF and/or any associated organization, and each of their respective past, present, and future officers, directors, employees, and volunteers, whether acting officially, or otherwise, resulting in my injury or illness stemming from my participation in the Programs. I understand that this waiver means that I give up my right to bring any claims including those for personal injuries death, disease or property losses, or any other loss, and give up any claim that I may have to seek damages, whether known or unknown, foreseen, or unforeseen. I further attest that I am over 18 years of age and possess the capacity to enter into the Waiver and Release Agreement.

SEVERABILITY: If any term or other provision of this Waiver and Release Agreement is determined by a court of competent jurisdiction to be invalid, illegal, or incapable of being enforced by any rule of law or public policy, all other terms, provisions, and conditions of the Waiver and Release Agreement shall nevertheless remain in full force and effect.
